Practices should ensure that their Policy and Procedure Manual accurately reflects the actual procedures carried out on a day-to-day basis.

Read each procedure and ask yourself and your staff:

* Is this how we actually do this process?
* Does the procedure need to be updated to say what we do?
* Should we do it the way the procedure states?
