Practice Information Sheet
A Practice Information Sheet is an important source of information about the practice that is easily accessible to patients.
It should contain at the minimum:
* Names of GP(s);
* Names of clinical staff working within the Practice;
* The Practice's address & telephone number;
* Consulting hours and arrangements and contact details for after hours care.
