Home and other visits

Criterion 1.1.3

Regular patients of our practice are able to obtain visits from a GP (where such visits are safe and reasonable), in their home, residential aged care facility, residential care facility or hospital, both within and outside normal opening hours.

Indicators

A	There is evidence that patients of our practice access home and other visits both within and outside normal opening hours (health records review, document review).  (Not applicable to After Hours Care services that make home and or other visits)
B	Our GP(s) and staff can describe our practices policy on home and other visits both within and outside normal opening hours, and the situations in which a visit is appropriate (interview).  (Not applicable to after hours care services that only provide consultations within a clinic)
C	Our practice has a written policy on home and other visits both within and outside normal opening hours (document review).  (Not applicable to after hours care services that only provide consultations within a clinic)
D	Our practice has used patient feedback to establish whether patients of our practice are aware of our arrangements for home and other visits both within and outside our normal opening hours (patient feedback). (Not applicable to after hours care services that only provide consultations within a clinic)
